Under 18s Consent Form

This form must be completed, signed and approved before a player under the age of 18 trains or competes in a Senior Competition or Higher Age Group.

There are two parts:

• Part one is completed by the player, covering their details, emergency contact, and acknowledgement of the additional risks of playing up.
• Part two is completed by a parent or other person with legal responsibility, giving consent and confirming they understand those risks.

HBL strongly recommends speaking to the club’s head coach and club welfare officer before a player takes part in a Senior Competition or Higher Age Group.
